1/15/2024 EnchantaVaughn Approved QMHP-A inquired about Kaila’s thoughts, feelings, and emotions. QMHP-A reinforced the importance of being accountable/responsible for her choices/decisions. QMHP-A inquired about Kaila’s struggles with paying her bills in a timely manner. QMHP-A reinforced the importance of taking her medication as prescribed as opposed to on an as needed basis. QMHP-A educated Kaila on the benefits of taking her psychotropic medication as prescribed (stabilizing her moods and overall mental health). QMHP-A inquired about Kaila’s inability to let go of individuals who contributes to her high levels of stress and depression. QMHP-A utilized a feelings chart to try and guide Kaila with verbalizing her feelings beyond the terms of stressed, a skill presented in previous sessions. QMHP-A prompted Kaila to utilize one or more of the coping skills she has been exposed to and processed with her if she has utilized any of them on her own and asked her to gauge how effective she felt any of them were. QMHP-A probed Kaila regarding her eating and sleeping habits and if they had improved since last week. QMHP-A prompted Kaila to develop a schedule for the week to assist her with accomplishing tasks and not procrastinating or abandoning tasks / appointments altogether. Kaila appeared to be receptive to QMHP-A’s interventions and services that were provided to her during the session. Kaila shared that she plans to save more money each month because she has to pay rent and will need to personal hygiene products and food for herself. She stated to QMHP-A She shared that she prefers living the way that she does because now she has no other choice, but to rely on herself. Kaila informed QMHP-A that she has been writing more in her journal, but she continues to have difficulty reading the entries. She admits that reading all that she has endured throughout life makes her sad and depressed. Kaila remained receptive to the IADLs being reinforced. She admits that she has difficulty being accountable/responsible for her choices and decisions. Kaila was receptive to building/maintaining healthy relationships. Kaila was able to create a schedule with the assistance of QMHP-A on what she needs to accomplish within the next week (i.e. two job searches a day and apartment searching at least two times within the week).
